Connect™ Platform

Coordination of care is increasingly critical to the health and well-being of members. Our Connect™ Platform links participating local pharmacists with health plans, prescribers and information designed to guide the process of care, so local pharmacists can be an active part of a member’s healthcare team. For health plans utilizing an internal MTM team to deliver services alongside our Personal Pharmacist™ Network, Connect can serve as a common documentation platform for centralized reporting. This advanced workflow, documentation and billing platform tracks MTM opportunities and performance in real time. Within your Client role, Connect gives you access to reporting, so you can track performance in aggregate or down to an individual pharmacy.

Insight™ Clinical Engine

Insight™ Clinical Engine

OutcomesMTM’s proprietary Insight Clinical Engine securely processes data provided by health plans and delivers actionable clinical information to network Medication Therapy Management (MTM) providers. Intervention prompts generated through our Targeted Intervention Program (TIP®), known as “TIPs”, support patient care and drive quality by focusing MTM activity on medication issues such as gaps in care, non-adherence, high risk medications and cost-saving opportunities.

Through these targeted interventions, local pharmacists can directly impact quality initiatives related to HEDIS and Medicare Star Ratings. The OutcomesMTM Clinical Services team maintains and develops TIP® protocols. TIPs deployed may be updated and/or customized according to your organization’s needs, new clinical guidelines, changes to existing quality metrics and evolving best practices.

The OutcomesMTM Connect™ Platform, including the Insight™ Clinical Engine, earned Certified status for information security by HITRUST, effective June 2019. The HITRUST CSF Certified status signifies our Connect Platform has met key regulations and industry-defined requirements and is appropriately managing risk. By including federal and state regulations, standards and frameworks and incorporating a risk-based approach, the HITRUST CSF helps organizations address challenges through a comprehensive and flexible framework of prescriptive and scalable security controls.
